New, Improved HCQ Survey

I corrected the Google Form so that each question is optional.

I meant to do that to begin with, but screwed it up.

Anyhow, the new Google Form doesn't require that you answer any of the questions before submitting.

The survey is anonymous, not even I know who responses are from.

It takes about 1-2 minutes to complete.

You can email the URL to anyone.
